A fill in the blank/check mark 1/2 page form for students of all ages to start learning personal responsibility! It's also an amazing way for you to document why a student is missing assignments when it comes time to talk to parents. :)

This goes well with my "MIA Request" form! While schools are moving to 1:1, not every student always has easy access to grades at home and frequently forget/lose passwords when they are at school at access grades. This is a great little form (4 per page) to let students know they are failing/close to failing as a grading period ends or a check approaches for performing arts/athletics eligibility! I fill them out for all my classes, make a copy for the kids and keep the originals for my records.

For those disruptive kids who don't quite get there are consequences for actions... When all roads of effort have not helped, allow the student to "choose" to fail and present this form, filled out and signed by the student, to Admin and his/her parents or guardians. This way, you have documentation to cover your actions and show that every attempt on your end to help the student has been exhausted.
