My fourth grade team is all about students making goals and achieving them. This Reading Goal set is one of the ways we make this happen. Sharing students with a team of teachers? I do. My team runs these two sheets front and back. We have parents fill these out at Meet the Teacher Night. Then we place them in a binder. We keep this binder by the phone we share. The log lets us see the topics that have been discussed with parents previously.
