I used this test as my sixth grade pre-test for the school year. It is worth 80 points (you can adjust how needed) and there are two portions to this assessment: Written & Performance/Listening. The written section has 20 multiple choice questions including questions on dynamic symbols, tempo, articulations, style markings, rhythms, note identifications, etc.. It will ask students to identify the meaning of the top and bottom numbers of five different time signatures. There are six fill-in-the-b
I used this when I needed a substitute lesson or gave a musical lesson. The worksheet is in order of the movie for students to follow along. It helped with accountability to keep their attention and guide them through the movie. This version goes with the 1999 Annie movie and has 15 questions.
This is a worksheet guide for the most recent Annie movie (2014) with Jamie Foxx and Cameron Diaz starring in this version. Great for students to use while watching the film.
From the Stomp Ultimate Collection DVD series- Nov. 2010 release date. Movie guide for Stomp Out Loud. (The last question is geared towards a garage band project.)
This written music/band test has three parts. The first part has 15 multiple choice questions about music symbols, style, note & rest identification, tempo, dynamics, key signature, and time signature. The second part is 10 matching questions on note values/identification. The last section has 8 short answer questions. This test was written to be 35 points, but you can adjust that if needed. The test is four pages but I included the answer key so you receive eight pages total.
This is a starting point if you need to create something to order t-shirts for a class or fundraiser. You will need to add in who they make checks payable to.
This is used for our middle school as an icebreaker during the first days of school for students to get to know each other and learn about the group. Students have to find the musician who has done certain things and get them to sign their paper. You can assign a time and give rewards to those who get the most signatures.

I use this chart, especially for beginning percussionists, for students to identify which timpani drum the student should tune the notes on. Please keep in mind, there may be slight variations with different age instruments, but this is a typical/standard range that students can use as a resource.
Students will add/subtract whole, quarter, half, eighth, dotted note values. Students will draw the note missing to complete the problem. 14 questions.

Good to use in December, especially on sub plans if you are playing this piece in a concert. Gives a background information and has ten fill-in-the-blank questions.
This written quiz has 10 matching questions, 12 note identification questions, 8 music notation questions, and 4 key signature questions. It is set to equal 38 points.
These are handouts I give out to students/parents at the beginning band meeting once they choose an instrument. All six typical starting instruments are included: flute, clarinet, alto saxophone, trumpet, trombone, and percussion. You may have to update which method book you use and any details you can add or adjust for what the parents will need to purchase specifically for each instrument.
