We had our students, that wanted to be a representative for student council, complete this form. You just need to change the school and you might modify some questions to reflect your school's beliefs.

I assign these on Google Classroom. Each student gets a copy to work on through the week. I only have 50 minutes, so I do these the first 5 minutes of class. I use it as quick review to keep skills fresh. I also might throw stuff in when I see weaknesses in the classroom. These aren't meant to introduce the skill, just to quick review. These are missing some days due to days off and snow days. You could modify the documents, as needed. I also use myself, or other teachers in the questions. Yo

I used this as a really quick pre, mid, and post assessment on my 5th graders. Ideally, they should have subjects and predicates down before 5th grade. Although, I've learned that isn't usually the case. This was really nice to see how to pull small-groups and how long we maybe needed to spend reviewing subjects and predicates. It also would be great for lower grades that are just starting this!

To introduce a new text structure I have students first respond to the writing prompt. Then I'm like BOOM you know this structure because you just created it. And then we find tricks to recognize the structure from their own writing.
