We had our students, that wanted to be a representative for student council, complete this form. You just need to change the school and you might modify some questions to reflect your school's beliefs.

I used this as a really quick pre, mid, and post assessment on my 5th graders. Ideally, they should have subjects and predicates down before 5th grade. Although, I've learned that isn't usually the case. This was really nice to see how to pull small-groups and how long we maybe needed to spend reviewing subjects and predicates. It also would be great for lower grades that are just starting this!

This is the guidelines we gave for our officers to apply for our student council.

On this template, I have 2 columns because I teach 5th in the morning and 6th in the afternoon. This template is better for people who teach a specific content area. I always keep this blank copy and then click "make a copy" for each week.

To introduce a new text structure I have students first respond to the writing prompt. Then I'm like BOOM you know this structure because you just created it. And then we find tricks to recognize the structure from their own writing.

I combined a few different key parts of a lesson, mostly from responsive classroom. I only teach reading and writing, so you could you'll want to modify for your needs.
