This is a Lab sheet that can be used for a variety of experiments. It is editable, so you can change it to fit any topic you need. Students have to make predictions, tell results, and tell whether it was a chemical or physical change. When I used it, I had six stations set up with a variety of tasks for students to perform and figure out if the change was physical or chemical. They would rotate stations and fill out all of the information for each section of the chart.
