This tool is for IEP teams to help determine the level of support a student may require. Made up of 4 categories outlining school performance, the team will rate the student on a 0-4 scale for each category. Once finished, a guide of how to use the tool after completing the scale is provided. This tool can assist in determining if/when a student may require one on one support and what type of support that may be (paraprofessional vs RBT). After a behavior referral, students are often expected to apologize. This worksheet allows for reflection on the behavioral incident, identifying how the individual felt, who was affected, how they can fix it, as well as how they will respond better next time. The file includes open ended short answer responses, as well as an adaptive version where the student can circle the response that is closest to the situation they had.

A momentary time sampling data collection sheet for 45 minutes that also includes yes/no minute by minute data collection for on task behavior, and a space to collect data based on directions followed within 10 seconds, and any outside factors that may have impacted the student being off task.

When given a behavior plan that says provide reinforcement on a variable 4 schedule, it can be hard to maintain that average of 4 in your head while doing all of your other jobs. These cards outline when to provide reinforcement based on variable ratio schedules 2-8. Cards can be individually cut out and put on a binder ring, or you can post the sheets as is within the classroom for staff to reference.

Partial interval data sheet broken into 15 minute increments for up to 4 behaviors. Behaviors can be identified and defined, and then tracked to determine if behaviors are specific to a time of day, a specific environment, as well as other patterns of behavior. Super teacher friendly for implementation in that it is less time consuming than frequency count data collection, but still gives a good picture of the behaviors. Great alternative to having teachers collect ABC data during the evaluation

This is an example and resource for student's who have Positive Behavior Support Plans. This can be helpful for substitutes, staff who may not work with the student frequently and need a refresher, as well as a reminder of expectations of the plan to ensure treatment does not drift over time.

“But they have a behavior plan….but it’s not working??” Using this sheet can help identify if the team needs more training, or if the plan is ineffective. It is an editable document that allows you to outline all the interventions within the behavior plan, collect data on the implementation of the plan, and debrief with the team.
