This Excel template is designed to be a quick and easy way to report the results of full grade speech screenings. Once the demographics have been added in, you can take your laptop with you to do your screenings. If you have no concerns document "pass" in each area. Likewise, if you have concerns you can quickly document your concern in the comment section. Likert calculator
This excel workbook is used to calculate a percentage using scores from a Likert scale from 0-5.
When opening this file, please enable macros. This will enable the calculator form to work properly allowing you to easily input your data.
You will receive a percentage based on the Likert Scale, and a percentage comparing a wrong answer to any write answer. You can assign any meaning to your scale but this is I typically use:
0=Wrong Answer
1=Full Model
2=Maximum cues
3=Moderate cues
4=Minimu

Teaching style
I strongly believe that my job as an SLP is to do as much for my client in a little time as possible. I like working with younger children because I feel that I have the greatest impact on their communication skills.
