About the store
I've been teaching since 2005. I have taught 1st and 2nd grade in the general education setting. I have taught 6th grade math for over ten years of my teaching history. After teaching regular and advanced math, I became a math specialist to tutor and guide fellow teachers on math best practices and now I work as a gifted support teacher where I specialize in lesson creation for gifted minds as well as catering to the social/emotional needs of gifted learners.

Being a case manager of an IEP/GIEP student can be challenging when it comes to gathering feedback from the general education teachers. This form allows that step to be easy for the teachers. You send this form out, they check the boxes for where the student rates in various categories, and finally, there is a place for comments. All areas that need to be filled out are "greyed" out to make it easy on the teacher as well.
