Being a case manager of an IEP/GIEP student can be challenging when it comes to gathering feedback from the general education teachers. This form allows that step to be easy for the teachers. You send this form out, they check the boxes for where the student rates in various categories, and finally, there is a place for comments. All areas that need to be filled out are "greyed" out to make it easy on the teacher as well.

Reflecting on learning is an important step in the process. This document allows reflection to occur for a group project that involved a presentation at the end. Of course, you can edit out sections that didn't pertain to you. Things are highlighted to show possible edits you may want to make to fit your classroom. This is designed to have students type into the document and shared via google docs. If you use this as a printed source, I suggest you make the boxes bigger for answers and possibly
