This is a simple editable check-in check-out Word Doc template. You can add or take away as many criteria as you'd like. Each item (on both check-in and check-out) are worth one point. At the bottom, is the end of week 'rubric,' where you should type in the total possible points; so when you print them off to write in for the student you just write in their total at the end of the week.
