This is a simple desk reminder for students who are pulled out for various SPED/ESE services. The reminder is meant to be put on their desk, you may laminate or cover entire reminder with clear packing tape. First page has a boys head on it and the second page has a girls face. This is a custom listing with ELL, Intervention, Speech on some cards and Math, Writing, Reading on the others. Still one page of boy images and the other of girl.

A simple referral template for documenting referral information for Response to Intervention (RtI).
