This is a form that you can prepare in advance for a parent teacher conference. There are nine spots for tests because we have 9 week cycles at my school. There is even a spot for you and the parent to sign so that you have documentation that you both spoke about the items on the form. Then store it in whatever binder you keep your parent communication in.
This activity allows students to match fractions, decimals and mixed numbers. It requires students to simplify or scale up to create equivalent forms. I also included the word form for each so that students who needs the extra practice have that available. Quick writing activity for students at the end of the year. It is designed to grades 4-8 as an easy way to share what they remember from the year or as a way to write to next years incoming students. Students can share tips for future students, their favorite memory and what their favorite class/teacher was that year.
