Included are the following: - a "simple" goal sheet: this sheet has a straight forward format for students who already know how to create an attainable, relevant, and clear goal for themselves - a "strategies" goal sheet: after creating a goal, this sheet requires students to consider and record strategies that will help them achieve their goal - a SMART goals sheet: this sheet gives students more prompting to follow the SMART goal process to create their behavioral goal

This includes both a pre- and post-assessment for the 6th Grade Common Core Ratios and Proportional Relationships domain. Every standard within this domain is represented in the assessment and real-world problems are included. The questions require students to show their work and to think through problems in a similar manner to questions on state tests.
