This form is a life saver! Every time a create one of my student's IEP's I also fill out this form. So much easier than going back through the entire 20+ page document. Easy access for classroom para-professionals, service providers and general education teachers. When I fill mine out, I bold all the students personal information and highlight the boxes with services the students receive and if their Tri or three year evaluation is due.
