This is a Recess Form I go over with my students at the beginning of the year. If they owe recess minutes for any reason, the form gets filled out. Under the comments section, either the teacher can write an explanation or the student can explain why they owed recess. The child must have their parent/guardian sign the form and have them return it the next day. I always make a copy prior to sending it home in case it magically gets "lost" I file the signed copy and use it if needed during parent-
