I had my students do a book recommendation/review on a book they had read recently. They used this template to help them organize their thoughts into the correct format. This packet can be used for independent work, literature circles, or small/large group discussions.
This is a student response packet for the novel The Sign of the Beaver by Elizabeth George Speare. Students can complete these independently or with a partner, and then meet together in a group to discuss. Each page has the same components:

This is a one-page brainstorming handout to help students plan a narrative based off from the text Alexander and the Terrible, Horrible, No Good, Very Bad Day by Judith Viorst. I have used these successfully with students. and parents love the communication. I often store these in a binder and have the student bring it back and forth between school and home.
Vocabulary practice for all 25 lessons from the Journeys 5th Grade Curriculum Resource. Each worksheet has the students write the definition, draw a picture, and use the word in a sentence. My students usually enjoyed this activity, as they were able to use their creativity with both the pictures and the sentences, all the while practicing the vocabulary words.
This is a simple form to communicate with parents regarding a student's late homework. Space to write what work was late, as well as a place to check if the work has now been completed, or if the student still needs to finish it at home. The bottom portion is for the parent to sign, indicating their awareness of the late work.
