All About You

This is a fun Google Form to have students complete. It has pictures and words and be used for multiple grade levels. It includes a mixture of interest questions (i.e. favorite color, favorite food, etc.), as well as open-ended questions (i.e. if you could live anywhere in the world, where would it be and why?). This is useful information for learning about your students, finding out their interests/preferences, as well as what type of reinforcers might work well for each student.
