This is a form that I created for use with my middle school students. It may not be appropriate for younger grades, but could possibly be of use for HS students. There are a few lines to fill in based on the individual student, but the rest is just checking boxes for strengths and concerns about the individual student. There is a third column with a list of tips for success.
I paired it with a Student Self-Reflection Form (also posted in my store). It gave parents the ability to see how stu
This is more of a "how to" backward design unit plan template. I also have a blank backward design unit plan template in my store. The different sections help keep you organized and help you create a complete unit plan. The guiding questions help you figure out what needs to be written in each section.

Implement this close reading bookmark in a mini lesson with students who need additional support in responding to the text they're reading. It has a list of sentence starters that help kick start those students who just don't know what to write.
This is a self-reflection form I had my middle school students fill out prior to holding parent-teacher conferences. This may not be appropriate for students below 4th grade, but may still be applicable to HS students. I paired it with my Parent-Teacher Conference Form (also posted in my store). By using both forms, parents got to see their child's opinion of his/her own performance and compare it with my opinion of his/her performance.
This is intended to use with your already existing classroom management system. Choose which step of your own system you'd like to assign this behavior reflection. The writing assignment can be used as an in class consequence or as a homework assignment. The more immediate the better. Conference briefly with the student after he/she has completed the reflection. Date and initial the bottom and keep in your behavior records for future reference.

These posters do not contain actual written targets; instead they contain space to write your own individual Common Core learning targets for your subject. Print the posters on cardstock and have them laminated for longer life. If you don't have a laminator, you can place them inside those clear plastic page protectors. Either way you can write on them with a dry-erase or wet-erase marker for easy clean off. Display the posters during your lessons to create a pu

Use these half sheet forms to encourage students to recommend the good books they have read (page turners) to other students and even staff! There are individual boxes to fill in the basic information about the book and a larger box for a brief explanation of why the book is being recommended. You could create a bulletin board in the classroom or even in the hallway, in order to display these book recommendations.
Use this weekly reflection sheet to promote self-reflection/assessment and metacognition among your students. It has a section for each day (M-F) and even a weekend wrap-up section. It uses rating scales and even has space for short answer responses. It is generic enough to be used with any subject area.
Make copies of this simple Sub Report Form to leave in your substitute folder when you're out sick. It has boxes with specific categories for your sub to leave you notes about how the day went. Make it easier on your sub, and leave him/her this structured handout for notes. Subs are more likely to leave you with detailed information if they have a tool to use.

This spreadsheet is for teachers to keep track of what students and groups have turned in, concerning the research project. It is specifically intended to be used with the research packet design that is also posted in my store.
Use this document with your students every time you visit the library, or even when you do book talks. Students can record basic information about the books they'd like to read. The next time you visit the library, they can use the list to remember what interested them previously. It is two pages, so you can print it to be double sided.
