The Smart Cookie

This is a an information sheet I created to keep all of my students' important information in one place. I have each family fill one out and then bind them into a book to keep near the phone in our classroom. It's an easy way to keep all the info at our fingertips! I have left this in a word document so that it can be edited as needed. I hope you find it helpful.
