I use this parental consent form once a student has been identified as needing Title 1 intervention services. It is important to keep an open line of communication with parents prior to beginning any services. I included a disclaimer at the bottom indicating that if the form is not returned (which unfortunately is a problem at my building), that students will be served right away. The form is editable so that you can include your schools information.
