This spreadsheet allows teachers (any level) to enter data for multiple students over the course of a year (by different classroom groups or different content areas for elementary) and a graph is created for each student for each quarter for each content area or classroom. There is also a 'master graph' for each class generate so that it is easy to notice students who are over or under performing the rest of the class so that appropriate interventions can be made. This Excel spreadsheet is set up t collect and graph pre-post test data for Elementary ages, Prek-5th (up to 5 classes for PreK and up to 6 classes of all other grade levels. As long as the same scale is used on pre-post, the chart and then graph will generate the % change for each student and then a composite for that class. The graphs are produced by grade level with each classroom represented. Great for guidance follow up, generating data to inform the counseling program and reporting data.
In attempting to make matches with mentors for students who need them most, I developed this rubric that assists in determining who might rise to the top of the 'wait list'. Certainly there are factors not included on this rubric that sometimes 'trump' them all. I use this to assist in this process being a little more objective. I send the rubric electronically to the teachers who work with the student most with the header information already completed (names, dates) and ask them to send it back
This is a basic behavior chart (a football chart for a desk) from which the data can be entered into a graphing worksheet (same file). This could be modified for a student's choice (not just football). There are graphs created for two quarters that include a goal or aim line and a trend line along with the student's data.
