This is a terrific handout for me that I always give out on the first day of school It is a one page questionnaire that tells me a lot about my new students. I can read through them and get a feel of how the culture of the class will be. After a couple weeks in the classroom when I have had the chance to learn my new students names, I look at these again. Do you need students to come in and take a test. I have often had large numbers of students who come in and need to make up an exam or to retest and during class I get so busy that I forget to remind them. What I do is print these out and fill in names and the make up test date. I will appoint a student to pass these out to the students they belong to.
