Set the Rule and then complete the Input/ Output Table. It can be used for addition, subtraction, multiplication, or division. This can be used with Input/ Output task cards or you can partly fill in the form your self to have the students figure out the rule and the missing numbers. Response to Intervention (RTI) documentation form
This form can easily be used by teachers or interventionists to document interventions and progress during Tier 1, Tier 2, or Tier 3 of the Response to Intervention (RTI) process.
