I found an RTI "If this... then this..." document for intervention instruction for Reading or ELA but never found one for Math. I put this together after college. It's not perfect or fool proof, but it's been effective in my experiences, and it does fill the void of an "If this...then this" piece for teaching math and doing RTI.

This form asks students to reflect on their progress in math over the course of a term, summarizing what they have learned and what they want to work on, and rating themselves on a scale of 1-5 on a number of important habits of excellent students. It is especially appropriate for advanced or gifted students.

Missing Assignment Google Form
The hyrbid teacher model has been overwhelming and I was trying to find a way for students to quickly let me know if they have completed any missing or late work.
