I use this Weekly Progress Report to let my parents know how their studetns is doing with Academics and Behavior. I put any weekly quiz grades and notes about what their student needs to work on in the Academic section. I then put their behavior for the week in the Behavior section. There is a place for the parent to request a conference if they need to. This was created with Microsoft Word so you can change it if you wish.

Weekly assignment sheets are given to students to help train them to be organized. It is also an excellent communication between teacher and parent. Because students have to get them signed, parents can't claim they didn't know about the test, quiz, whatever! Use year after year, just make changes as needed. Ms. Barkdoll promises parents on Open House night that their children will finally learn to be organized! Also serves as a lesson plan for my principal.
