Weekly Progress Reports
I use this Weekly Progress Report to let my parents know how their studetns is doing with Academics and Behavior. I put any weekly quiz grades and notes about what their student needs to work on in the Academic section. I then put their behavior for the week in the Behavior section. There is a place for the parent to request a conference if they need to. This was created with Microsoft Word so you can change it if you wish.

This is the format I use to send home a weekly newsletter. The left column I put daily lesson numbers, quizzes, and special events. In the right column I inform parents of basic curriculum for the week or any other information they may need.

Weekly Assignment Sheet
Weekly assignment sheets are given to students to help train them to be organized. It is also an excellent communication between teacher and parent. Because students have to get them signed, parents can't claim they didn't know about the test, quiz, whatever! Use year after year, just make changes as needed. Ms. Barkdoll promises parents on Open House night that their children will finally learn to be organized! Also serves as a lesson plan for my principal.

Customizable Homework Pass for Students (FREE)
These homework passes were created to be customized by the teacher instead of using a "generic" one. There are three homework passes on the page to be printed out and cut. This is a great sheet to slide into a folder with a clear plastic pouch cover. Students can use a dry erase to check off the items they put in their backpacks to go home so nothing gets left behind in order to increase personal responsibility and organization skills. Can be modified.

Bears in the Cave - Student Tracking Form - Numbers 1-20
I keep this at my guided reading table. Whenever we play "Bears in the Cave" I just do a quick check next to the numbers the students get correct. This helps me to know where the students are, but most importantly it helps me to quickly group students for math centers. I can have my higher students (the ones in the teens-twenties) quiz each other as a center. I can pull my lowest ones and work on them one on one.

Behavior Intervention Chart
