Students record their own quiz scores on this tracker so that they can see their progress throughout the year. It also shows at a glance what topics they need to spend more time on and which topics they have mastered already. This is a great document to refer to in parent/teacher conferences to show parents what their students need the most help on.

Student Reading Record Sheet

I use this form for my students to record their Accelerated Reader (A.R.) quizzes on. At the top of the sheet is a place for the teacher to fill out the student's weekly point goal, six weeks point goals and level goal, book levels they are allowed to read, and what types of books they can read. Students are able to determine on their own whether or not they have met their ar goals.

This document was created as an individual student sheet. The teacher can record points the student has earned for each category. The categories included are participation, homework, quiz, bell work, and tests. I have also included a column for the grade and extra credit. You may change the categories to accommodate for your classroom grading and activities. I have also included a place to record absences, restroom passes, and in school suspension days. I designed it so that you can place

Hall Pass Permission Slip - Add accountability to hall pass requests!

HALL PASS PERMISSION SLIPS Make students EARN the right to use the hall pass! They must write a paragraph before they can leave. In my Spanish class, they simply use current vocabulary and grammar in their paragraph. In other content areas, they might make a summary of the day's lesson or of the current unit of study. Since I started using this hall pass idea, the number of students who try to take advantage of the system has decreased dramatically! Students who used to "need" to leave

This is a simple weekly agenda sheet for primary school kids to use. I call it the A.B.C. (Agenda & Behavior Chart). It has space for the parent to initial when the homework is complete. It also contains a daily section for the child's behavior. I print several copies at the beginning of the school year, write the individual weeks on each page, then make copies for my students each week to put in their binders. On Friday, I put a large check on the agenda sheet if it has been completed for t

This is my most popular lesson plan template. I like that this one has the basic framework of each period listed on the left and then blank space for each day to allow for notes & reminders. My completed version comes out to two landscape pages, but it will adjust to the content you enter. This is a Word document that can be modified to fit your needs. Copyright Geaux Teach Online Store. "Find a Partner" student record sheet

This is a partner sheet (inspired by Kagan's "Find a Partner") that I use in my class to help establish partners among my students. Initially, I used Kagan's structure of "Mix-Music-Meet" to have students partner up, and then I tell them which partner that classmate is (like their soccer partner or cooking partner). To signal they are done with whatever I've asked them to do with their partner, they simply "play soccer" or "cook" with their partner until nearly everyone is finished.
