All those forms we are given for assessments can be draining to fill out. Here is a simplified form to use as a quick look at how they are doing! I've used these in my classroom personally and I love it! UPDATE: After i posted this product for some reason and it showed it without color on the form and different fonts (weird), I downloaded it from the site and one its downloaded it looks exactly how I made it. Sorry it doesn't look great on the preview.
