I created an easy to edit and easy to use form to send home daily communication to parents via pen and paper, typed and printed or conveniently emailed to parents. Directions are included in how to fill out and email easily.
The form is edited through powerpoint and filled out on Adobe. Make sure you have access to these formats to ensure proper use.

Self-Monitoring Behavior Charts
Two different levels of self-monitoring behavior charts.
How to use:
I sit down with each student and we chose 4 behaviors to monitor throughout the day. They also chose the reinforcer they will be working towards if they earn the necessary points.
After each segment, a paraprofessional or myself review if they met this expectation. At the end of day, if they earned enough points then they get their reward!

EDITABLE Behavior Contract for Lower Elementary Grades FREE!!
I use this behavior contract with my students who need just a little bit more motivation. It is very effective if used consistently and with parent involvement. I usually have the students work towards a personal goal/reward to motivate them to follow the tasks listed on their contract (i.e.lunch with teacher, extra iPad time, etc.)
