I use this chart to collect data and trials on students who self initiate or follow my request to initiate taking some sort of a sensory break or when a coping skill is needed. Can be modified/changed to address more specific strategies and/or behaviors.

Reading Strategy Post It Note Chart
This chart is a great way for students to keep track of the reading strategies that they are working on and those that have been mastered.
The teacher will write the strategy the student is working towards mastering on a post it note on place on the chart. The chart is stored in my classroom in each student's (read to self) bookbag. It is a handy reminder to students of strategies they can try when reading individually.
