This is a great tool I use for kids who need a little extra help with being held accountable for their grades in class. Instead of having one after the other phone conversations with parents, I use this sheet everyday to check off if homework has been handed in or assigned, and at the end of the week list grades on quizzes or tests. Parents can sign the bottom of the sheet to let me know they have seen the report.

Use this page weekly to keep notes on your students. You can add the I can to the top and then check mark if they got it, give a quick note of how they almost have it, or write a note of what they need to still work on.
