Math Reflection Response Journals and Exit Tickets
Included are 4 versions of my math reflection journals and exit tickets. They are open-ended questions asking students about the skills taught that day. Use their responses to tailor your instruction and pinpoint math skills that students need the most help with. Use their responses as extra writing instruction that students desperately need.

Assessment Binder Student Graph Reflection Chart
Students learn from reflecting on their progress by graphing their success and recording what they did to earn their score or what they could do differently next time. Students use one for every subject and set goals for what they hope to accomplish each month. Students take home their assessment folders with their graded work behind each graph for students to review with parents. I use their binders during conferences.

Course Reflection Sheet
**Suitable for middle/high school classes**
Receive feedback from your students at the end of the year to reflect on your practices.
The students rate themselves and the teacher. Then, the course reflection asks the students what they liked most about the course and what lesson they would like the teacher to change. I also included a question about advice for next year's students--it's something fun to include in the beginning of the year.

Do Now / Exit Slip - Formative & Summative Assessment - good for all subjects!
This is a great graphic organizer (front and back) with space for a "DO NOW" and Exit Slip question Monday-Friday. The back of the sheet also includes a self-reflection on the week for effort and behavior. This can be used for a weekly grade and as both a formative and summative assessment of learning. I found it a very useful way to start my middle school classes, but could see this as something to implement from grades 3-12.
