I utilize this template to recreate "mini IEPs" for my general education coworkers. After each IEP meeting, I create this sheet (or update the current one) and then pass out to all teachers who see the child throughout the day so that they know the child's strengths, needs, goals, accommodations, services, and reinforcements.
