On this AR Reading Log there is a place for them to write the Book Title and Author's Last Name. They are to check off when they have read it 2x's and when they took a test. After they take an AR quiz, they then fill out how many points they earned, their percent, and have their parents inital their reading log.

There are a total of 13 chapters in this book. I choose to split the read aloud into 2 weeks. Chapters 1-6 were read in week 1. Chapters 7-13 were read in week 2. At the end of each week students were given a vocabulary quiz on the chapters that were read and discussed. We practiced the vocabulary words throughout the week. This document is the vocabulary quiz for week 1. The vocabulary quiz for week 2 will be uploaded as well. Enjoy!

I use this form for my students to record their Accelerated Reader (A.R.) quizzes on. At the top of the sheet is a place for the teacher to fill out the student's weekly point goal, six weeks point goals and level goal, book levels they are allowed to read, and what types of books they can read.
