This is a weekly behavior chart, used by teacher and child. Teacher can fill it out or by the student. The perameters can be changed to meet the student but I devised a way to save paper by doing it weekly, and also see patterns that occur over the week/month. Parents have a place to sign and put comments, as well as a place for the teacher.

These categories align with the CCSS ELA Module Assessments we had in Florida this past year and go along with the grading rubrics for those assessments. The primary and intermediate had slightly different rubrics, but categories were the same so this is aimed at intermediate since I used it with my 3rd graders, but may work just fine for primary, too. Includes basics like preparedness and punctuality, but also quantifies some of the "stickier" areas of behavior in class, like openness to learning and presence in class.
