Quick and simple behavior note for quick communication with parents about a student's behavior. This one page document is fast to fill out. Student's name, the date and then simply check the boxes of the poor choices or behaviors that occurred that day. The note has a parent signature at the bottom. How To Determine Readability
Utilizes Gunning Fog method. Simple worksheet with an example calculation that can be given to students or adults to calculate an approximate grade level when using a sample of text. Disclaimer: this is an abbreviated version ideal for students or someone just learning to calculate readibility.
