Teacher-Staff Survey Template

This document is intended to be a base format for administrators and supervisors to use in collecting data. It is recommended that questions be theme based for ease of categorizing responses. Adding questions is possible but not suggested as busy teachers traditionally resent required paperwork to begin with. Quick and simple will result in higher number of responses. Handing out one survey per month encourages feedback without increasing the work burden. This template could easily be modif

This is a simple document that I created in order to help me keep track of all of the Student Study team meetings (SST's) I had to go to through out the school year. As a special education teacher, resource, I was on the second tier of the RTI program at my school and therefore went to SST's for any student at any grade level. Because we had so many, I needed a form to keep track of the who, what, and whens: the people involved, the interventions already tried, when the meeting happened, the nex

Student Observation Log

This is an EXTREMELY simple form I keep in my binders for each class. There is a copy of this form for each student. I keep a clipboard of labels nearby during class, and when I note a behavior that I want to record, or if there is an incident in class, I record it on a label. Later when I have time, I stick the label on the observation form. Now my documentation is a piece of cake. If I need to talk to parents or administrators about a student's behavior in class, I just have to look at my log.

I have finally developed one that helps me monitor progress in a more effective and organized manner. I have included a sample goal with benchmarks in this editable document.
