Our district requires teachers to take documentation that they are providing accommodations to students. This documentation is brought or given to the 504 chairperson prior to the committee meeting with parents or guardians. This documentation includes: 1. Student Identification Header Student name, class name, teacher name, student grade (9, 10, 11, 12), and Class Period 1-7 for teachers to keep track of which student the documentation is for. 2. Documentation chart for preferential seating
