Cut the form in half to send home with the student (or leave at the day care / preschool). Or log your notes on the computer then email or print for parents. The form is in DOCX format so you can edit as needed. Fill out the "Today, we worked on..." section with brief descriptions of the student's goals (e.g., making the /s/ sound)

This is a template that you can use for sending home notes to your tutoring kids. All you need to do is print it out, or fill it out on the computer and then send it home at the end of each of your tutoring sessions. I use this for my students that have action plans. I use the calendars to jot notes down about what we worked on that day. I will write the minutes, type of intervention, and if it was successful. I use the assessment and score to give check point data. The interventions section is where I write all the different resources we used together during the month. This is usually an ongoing list.
