I created this slip for students who have problems arriving on time, staying on-task and completing in class work. A staff member will fill out this form (This is a .doc. You can add/delete items in the document) with the student's name, parent/guardian's name, and fill out the student's classes, and teacher name and contact number. I use this behavior contract with my students who need just a little bit more motivation. It is very effective if used consistently and with parent involvement. I usually have the students work towards a personal goal/reward to motivate them to follow the tasks listed on their contract (i.e.lunch with teacher, extra iPad time, etc.) I truly hope this product works for the needs of your class! ** If you do not have the fonts downloaded on your computer, you can download them from Hello Fonts

This is a basic skills based report card that I use for parent teacher conferences. It covers skills in the areas of reading, writing, math, science, social studies, speaking and listening, character development and learning abilities. It is simple to use. I often found that the marks were too vague on forms I have seen in the past and this set of marks makes giving information more accurate. Teachers can use this to help motive students to reach a desired goal. It could be for work completion or a desired behavior, i.e. stay in their seat, raising their hand to talk. As students complete their work, or are caught doing the desired behavior, they receive a stamp or sticker to fill each circle.
