This report card is fully aligned with the Common Core Standards. ELA and Mathematics sections are directly from the Common Core. Acuity Report for Parents for 5th Graders
A report to send home after each Acuity Test that states what their Acuity scores are for test A, B, and C. The report also has a section for the student's previous year Acuity C and ISTEP+ scores. I normally send home student work once a week. In that folder I post this form, and keep it there for the grading period. Students set goals in AR and AM for each grading period.
