EDITABLE Behavior Contract for Lower Elementary Grades FREE!!
I use this behavior contract with my students who need just a little bit more motivation. It is very effective if used consistently and with parent involvement. I usually have the students work towards a personal goal/reward to motivate them to follow the tasks listed on their contract (i.e.lunch with teacher, extra iPad time, etc.)

I created this Check In Check Out Point Sheet for the use in my 1st grade classroom. This can be used as a daily behavior chart for students who need positive accountability, structure, and feedback. Students get to choose their goal for the day, as well as their incentive. Parents love the daily accountability as well. This is very easy for the classroom teacher to manage as well.

Daily Behavior Scale for ADHD
This behavior rating scale was designed for a student who has already been diagnosed with ADHD or has the same symptoms as ADHD. It isn't a screener to identify possible ADHD.
I fit 2 days per page, but if you set your printer to print on both sides of the pages, you can fit 4 on one sheet of paper.
