I will begin my 10th year of teaching in August 2015. I have taught 6th, 7th and 8th grade math and will be teaching 5th grade math beginning this year.
Math test corrections form, with template for tests that include the coordinate system, graphs and number lines. Students are asked to redo the missed problem, and then reflect on their mistakes. In my class, students complete the test corrections to earn half of the points back that were lost, up to an 85%. I have also had students complete this form before taking a re-test to earn points back.

This is a list of various strategies that students can use to review math concepts. In my class, I pass this out at the beginning of the year and students use it in conjunction with the study log to prepare for tests and quizzes during the year. It includes blank spaces for students to record additional strategies independently or as a class.
I use this questionnaire throughout the year for students to reflect on their math skills. I give it as homework the first day, and then ask students to revise it once a quarter (using a different color each time) to see if there have been any changes. It is a quick and easy way to keep me tuned in to how students are feeling about my class, and encourages them to reflect on behaviors that are important for their success. This study log helps students build good study habits by studying on multiple occasions during a week, rather than waiting until the last minute to cram. Students need to complete one hour (total) of studying in the week leading up to the assessment. They record their time spent studying, what they focused on and which study strategies they used. Are the rumors true? I use this activity in my classroom during the first week of school to see what my incoming middle schoolers are concerned/excited about. Using these sheets, students can anonymously ask about lockers, schedules, lunch time, social interactions, etc. Each student fills out one or more sheet.
