A copy & paste activity and reference chart for students who struggle with blurting personal or inappropriate information aloud. Some students naturally have a filter, but others do not. Great for kids with social skills difficulty, autism, and communication disorders.

Use this sheet to record how much time a student spends on-task. Space to write the activity, and to mark on-task/not on-task each minute. Arranged into 4-fifteen minute observation periods. I use this once a day, when I want to show how attentive (or inattentive) a student is.
