Let's be honest.. between teaching and managing a classroom, it's difficult to find the time to sit down with every student and check in on them. I sent this weekly check-in form out every Friday to see how my students' week went. I'd read these over Sunday coffee or early Monday morning, and address anything I needed to. One way students can advocate for themselves in the classroom is by asking for a break! We want to intervene with students early on to minimize disturbance, outbreaks, and student distress. You can place this printable in your calm corner, and/or print and give these to every student. My students would use non-verbal cues to let me know they were making a choice board break pick, and it would not interrupt the class. Enjoy!

This resource is great to print, laminate, and have copies of around the classroom. My SPED students and oppositional defiant students thrived with this graphic organizer. After teaching a lesson, I'd do a lap around the classroom and fill these out for students. By learning student motivators early on, this is a quick and easy way to get students going on their independent practice.
