This is a detailed PBS plan template with simple guides under the categories (ie. "Teaching Strategies: How will the replacement behavior be taught and by whom?"), so a background in PBS is not necessary to understand the form. It is 2 pages total (about 1.5 printed) with room for notes. This template is based on information from "Behavior Management: Principles and Practices of Positive Behavior Supports" By: Wheeler, John J., and David Dean Richey (2010).
Since I've gotten so many people interested in my PBIS plan, I've decided to share my Behavior Contracts. At my school, we implement a BIP. If that doesn't work, or if the student's behavior impacts others (such as physical aggression or bullying), or their behavior pervasively changes the climate of a classroom, we implement a behavior contract. I've included in-depth steps and examples for the team to examine behavior and design solutions. This is NOT the time for RTI and listing intervention
