I use this checklist in my classroom. When my students write journals, they must use the checklist before they turn their journal in. I then use this as a rubric taking 5 points off for each area they did not do.
I made this responsibility checklist for my students to evaluate how they are doing on being responsible. Then they had to read and sign the responsibility contract. I kept a checklist with their names and list each of these areas so I could track how they were doing. Each week they would get a responsibility grade.
To control classroom pencil use in my classroom. I decided to make this sheet. I keep it on a small clip board near the pencil sharpener. I have a bucket for sharp pencils and a bucket for unsharpened pencils. I made this form for a few of my students who were having trouble with leaving their work at school or not doing what was written in their planners. They had to mark that they checked their folder, write down the number of unfinished work pages they had left, and then have their parents sign it at home. In the morning, they turned in their sheets and then had me sign their folder. I would reward them for each day they completed their chart.
