Download, change to fit your student's name, skills to be worked on, and specific schedule. Students would use this to self-monitor themselves during each subject throughout the day by putting and X or red mark if they didn't do it and a star or green mark if they did do it. The teacher uses the last column to tally how many verbal and nonverbal prompts the student needed during the subject.

Download this editable form and make any changes necessary to fit the needs of your classroom and students. After filling out this form for each student, MAKE A COPY OF IT. Download, change wording to fit your needs, and print one copy for each student on a colored piece of paper. Use this to keep in take home folders for families to sign once they have viewed the contents of the folder. I like use the word 'families' intentionally because not all kids are assumed to live with a parent, or they do live with parents but someone else in the family is the primary caregiver. However, you can change the wording to fit your population of students.
