This is a great resource to help students set goals! I have used this at the start of the year when setting goals with students. They keep this in a safe place like their student folders, binder, or planner (etc). At a later date, we revisit their goal to see if they have met their goal or if they are still working on it. The students love tracking their progress and reflecting on how they are progressing!

This is a behavior plan that was designed to be used for one student. The teacher would fill out one paper per day for the student. There is a scale at the bottom of the paper to identify how the student has performed within each subject hour o of the school day. It also contains a goal to set with your student.
