This is a behavior plan that was created to be used for one student. There are 6 behavior plans on the paper. The teacher would cut out the plans out and use one for each day. This plan was designed to be used and initiated with the student. The teacher would create a goal with the student, discuss behavior expectations, and tape a plan on the students' desk each day. The student would receive a star in each box that he/she meets the expectations for that hour. The teacher can use star stickers

This is how I keep track of students' grades, participation, and reading / math data. This is uploaded in a powerpoint, so that you may enter your subject and student names.
