Addition, Subtraction, and word problem assessment. This test requires regrouping, explaining whether there is extra information in a problem, and a constructed response question. This is the Eureka Math module 6, 3rd grade end of module practice test. If your students can pass this, they will be successful on the EOM assessment for Module 6.
You could also use it for a pre-test for 2nd grade or a review for 4th grade.

3rd grade mid-module practice test for unit 5- Fractions as numbers on a number line
This Common Core practice test follows the actual mid-module assessment.
You could use this as a pre-test or as a review right before the mid module.
If your students can do well on this practice, they will do well on the actual Common Core mid module.

This test directly practices the end of module math EOM module 5 grade 3 assessment task.
Use it for a pre-test, a practice test, or as more guided practice in class.
This performance task assesses weight, mass, and capacity. I would recommend it be used for 3rd-6th grades. I used it to assess my Special Education students since they perform better when it is not a "test" they are taking.
These task cards cover decimals to the hundredths, percents, and fractions. You could laminate and use these in centers, or you could just use the problems on your assessments.
Students will need to convert fractions to percents, add and subtract percentages, figure out a sale price given reg price and discount, figure out what percentage the cost the tip was of a meal was, compare fractions to decimals and percents, and more!!

This test was written for 4th graders enrolled in Special Education. However, it can be used in 3rd-5th grades as special education or regular education. The only reason it is written for special education is because most questions have cues or "hints" to help the student solve the problems. You can easily white out, or cover the hints before copying.
Ordering numbers, rounding numbers, and a constructed response question are all included.

This activity focuses on the types of triangles- scalene, isosceles, equilateral, right, acute, and obtuse. There are a few questions that ask the student to measure a line segment on the triangle. This helps the student understand WHY the triangle is the type that it is. I used this in pairs, but it can be used in stations, learning centers, or even independently. Just copy the 2 pages of triangles and have the students cut them out, or you could cut them out and laminate them so that you can u
Your students have been transformed into goats. They must design several gardens in which to live. By identifying the perimeter and area, students will resolve which garden would be best to live in. Students must write the equation for area and perimeter and solve for each. This is a fun way to assess area and perimeter or use as an extra credit project.
A station activity I created for my students to use in teams of 2-5. This fraction activity includes improper fractions which can easily be removed if you just want to use proper fractions. The directions are for 2 teams to play, but this could be used individually or even whole class. There is an answer key and instructions on how to play included. Also included are the fraction cards.
This 4th grade assessment covers multiplication- 2 digit by 2 digit, 3 digit by 1, and 2 digit, 4 digit by 1 and 2 digit, properties of multiplication, word problems, estimating, and a multi-step constructed response assessment. An answer sheet for the students to write their answers on is provided as well.
This is a 4th grade fractions test. It has been modified to more closely resemble the LAA2 Special Education test in Louisiana. However, it can be used for any teacher, Special Education or Regular Education. The questions are very similar to the regular test, but there is more white space, only 1 column and not as many higher order thinking questions. This assessment could also be used to test 3rd graders on fractions.
This is a 4th grade, common core patterns test. It does not include a constructed response, that is why it is priced lower than my other assessments. It is an 8 question assessment that takes up 6 pages. This test includes hints on various questions for those students who need cues. You can always white them out before copying the assessment.
This could also be used as a practice assignment.

This is the fractions and decimals test I give to my 4th grade Special Education Students. It includes a constructed response question. This test could be used for regular ed students as well.
This is a test I modified for my students with special needs. The questions are exactly the same, there is just more white space and fewer questions per page. This could easily be used for regular ed.
